Child Care
PFCC benefits are available if a caretaker is participating in one or more of the following approved activities:
(a) Paid employment on a full-time or part-time basis.
(b) A training or education activity that prepares the caretaker for paid employment.
(c) Requirements set forth as a result of participation in Ohio works first (OWF) or the supplemental nutrition assistance program (SNAP).
Income
The family must meet the income requirements:
(a) Initially, a caretaker may be eligible for publicly funded child care if the family's gross monthly
income is at or below one hundred thirty per cent (130%) of the federal poverty level (FPL).
(b) On-going eligibility may be maintained if the family's gross monthly income is at or below
three hundred per cent (300%) of the FPL.

Application, Reapplication and Changes
The application process shall be considered complete when the caretaker has submitted:
(a) The JFS 01138 with the caretaker's name, address, signature and all information about the
caretaker's household completed.
(b) Verification of citizenship or qualified alien status for children in need of care.
(c) Verification of income for all household members pursuant to rule 5101:2-16-03 of the
Administrative Code.
(d) Verification of a qualifying activity for all caretakers.
(e) The name and address of an eligible provider chosen for each child in need of care.
Applications may also be mailed to the Mahoning County Department of Job and Family Services or faxed to 330-884.6165.
Forty-five days prior to the end of the current eligibility period, a JFS 01124 shall be mailed to the Caretaker by the state. If you did not receive your redetermination application, please click here JFS 01124
The caretaker shall notify the county agency regarding any change that impacts their child's current authorization for care prior to or within the service week that the new authorization is needed; a service week starts at twelve a.m. on Sunday and ends at eleven fifty-nine p.m. on Saturday. Provider Program
A resident of Ohio who wishes to become a licensed child care provider may visit the Oho Department of Job and Family Services website and click on Child Care at the top of the home page.
Transportation
NET (Non-Emergency Transportation) provides transportation to medically necessary appointments, such as renal dialysis, chemotherapy, radiation, as well as routine doctor appointments.
